ADDENDUM #1 HAS BEEN ISSUED AND MAY BE DOWNLOADED BELOW OR PICKED UP IN THE PURCHASING DEPT., RM 207, 820 MERCER ST., CHERRY HILL, NJ

BID OPENING DATE HAS BEEN CHANGED TO FEBRUARY 21, 2019, 11:00 A.M. PREVAILING TIME.  NEW BID FORM HAS BEEN PROVIDED WITH CORRECTION.

The project consists of improvements to Cherry Hill Blvd between NJSH Rt 38 & Monmouth Dr.  

The roadway improvements will include, but not limited to the following:
1. Variable depth milling up to a 3" max of the existing hot mix asphalt.  Contractor to install 2" of hot mix asphalt surface course 9.5M64 to match existing elevations unless otherwise indicated on the plans.
2. Hot Mix Asphalt Pavement Repair as directed by the Engineer after milling.
3. The installation of concrete vertical curb, concrete sidewalk, concrete driveways, & other incidental work as directed by the Engineer.
4. The installation of ADA compliant curb ramps at various locations as directed by the engineer.  Separate payment will not be made for truncated domes, brick pavers, installation of concrete sidewalk necessary for transition to the ramp, up to 30 linear ft of concrete curb, concrete headers, hot mix asphalt pavement repair associated with curb installation, the ramp landing or other items associated with the construction of the ADA compliant curb ramps.  However, all costs shall be included in the unit price bid of the item ADA Curb Ramp,
5. All related incidental work, including earthwork, traffic control, dust control, soil erosion & sediment control, cleaning & restoration.
6. Sanitary manholes are to be rehabilitated & made waterproof along with ladder rung replacement as noted in the plans & specifications.
7. Reset manholes, castings & vent/valve boxes.
8. Replace existing curb inlet head pieces with Eco Type "N" head pieces & existing inlet grates with bicycle safe grates.

Qualifications:
The bidder should be prequalified under both a paving (class code 2 and/or class code 3) and concrete DOT Class Code as listed below, or any sub code thereof. If the the bidder is not prequalified in both a paving and concrete NJDOT class code, or any sub code thereof, the bidder must be prequalified under one of the following paving or concrete DOT class codes and must list a subcontractor who is prequalified in the other specialty, providing both paving and concrete pre-qualifications between the bidder and subcontractor combined.

NJDOT Class Code 2- Paving
NJDOT Class Code 3- Grading & Paving
NJDOT Class Code 8- General Concrete

The bidder MUST submit with the bid written proof that each GC is prequalified and/or classified in accordance with the statutes, rules, and/or regulations promulgated by the NJDOT as applicable in the categories listed above.

Special Requirements:
The contractor is responsible for verifying the location of all utilities & for scheduling all work involved with utilities directly. All costs related to his coordination shall be included in the various items of his bid.

Contractor shall ensure residents/businesses have access to their homes/businesses during construction. This work shall be included in the various items in the proposal.

Miscellaneous:
The applicant/proposer shall submit 1 original with original signatures marked "ORIGINAL" & should submit 2 complete & exact copies of the original marked "COPY" of his/her proposal & should submit an electronic copy on a CD or flash drive.
